Municipal Information Form Submission to Municipality

The Alcohol and Gaming Commission of Ontario (AGCO) is responsible for administering the Liquor Licence and Control Act, 2019 (LLCA) and issuing Liquor Sales Licences. As part of the application process to obtain your Liquor Sales Licence with the AGCO, you are also required to submit your Municipal Information Form to the City for completion by the City Clerk.

Please note that this form is made directly to the Licensing Clerk on behalf of the City Clerk and is circulated to the Fire Department, Building Division and Public Health for approval. Once all approvals have been received by the Licensing Clerk, the City Clerk will sign off on the Municipal Information Form which will then be returned, along with all Agency Letters of Approval, to the applicant.

The applicant must then upload the Municipal Information Form and all Agency Letters of Approval to the iAGCO portal.
